Orthodontic Benefits

Your Willamette Dental plan provides orthodontia coverage for the entire family. Unlike other fee-for-service dental plans, we provide benefits for your orthodontic treatment when it is coordinated and provided by a Willamette Dental dentist.

Pre-Orthodontic Service Co-payments

Prior to undergoing orthodontic treatment, patients are required to first meet with their dentist, who will refer them to a Willamette Dental orthodontist for a pre-orthodontic consultation.

The pre-orthodontic service co-pays are as follows:

  • Initial orthodontic exam: $25.00
  • Study models and X-rays: $125.00
  • Case presentation: $0.00

Orthodontic Treatment Plan

Your Willamette Dental orthodontist will prepare a treatment plan prior to providing services. The co-payment is determined by the extent of the treatment rendered. The amount listed for Comprehensive Orthodontic Services in your Summary of Benefits indicates the maximum amount, plus office visit charges, that a patient will be required to pay.

Partial orthodontia treatments will be pro-rated based upon the amount of treatment required. A portion of the orthodontic co-payment is required at the banding appointment. The balance may be paid in monthly installments.

Takeover of Existing Orthodontic Treatment

Orthodontic treatment already in progress prior to enrolling in the Willamette Dental plan will be pro-rated according to the extent of orthodontic services provided by Willamette Dental to complete the treatment.

Orthodontic Limitations

Please refer to your Certificate of Coverage for orthodontic treatment benefits, eligibility, and possible waiting periods.

If coverage terminates prior to completion of orthodontic treatment, there may be additional charges for orthodontic services rendered if treatment continues after the termination of coverage.

Continuing orthodontic treatment will be pro-rated on Willamette Dental's fee-for-service rates. If orthodontic treatment benefits terminate before the end of the prescribed treatment period, benefits will continue through the end of the month in which the orthodontic coverage was terminated.

Provider Services

Implant Services

Implants have become an accepted standard of care for the replacement of missing teeth. Patients with healthy gums, good bone structure, and excellent oral hygiene habits are usually excellent candidates to receive dental implants.

Dental implants are titanium metal posts surgically placed in the bone. Implants fuse to surrounding bone tissue and offer a secure foundation to support an individual missing tooth and/or dentures. Advances in technology, surgical procedures, and restorative techniques have made implants highly successful and relatively easy to place.

Willamette Dental implant services are limited to surgical placement of implants where the bone and soft tissues are sound and healthy. In situations where bone and tissues are less than ideal for implant procedures, additional surgeries may be required to improve the site in order to support an implant. Additional surgery may include additional costs to the patient.

The treating Willamette Dental dentist reserves the right to determine the suitability of implant therapy and whither therapy will be initiated.

Implant services are offered through Willamette Dental providers at a reduced fee and are not a covered benefit under Willamette Dental of Washington, Inc. The services and fees are available only when a Willamette Dental general dentist or specialists performs the entire treatment. Patients receiving treatment for Willamette Dental implant services will be responsible for all fees.
